Is this for the 14 Plus or the 14 Pro?

The listing name does not settle it. The two use different arrays — two lens openings on the Plus, three on the Pro — so message us with your model and we will confirm what is in stock before you order.

Do I have to replace the whole camera?

Usually not. If photos are hazy or flared but the camera still focuses and shoots, the damage is in the outer cover glass and replacing that alone restores image quality.
