Can I just peel the old back glass off?

No. On the Pro models it is bonded to the frame, so it has to be broken out and the frame cleared of every fragment and all the old adhesive. A laser separator makes this a manageable job; without one, expect heat, cutting wire and a long session.

Will the phone still be water resistant?

Treat any opened phone as no longer sealed to its factory rating. Fitting fresh adhesive all the way round restores a good physical seal, but the original certification does not carry over.

Do I need to match the colour exactly?

For resale value, yes — the rear panel is the largest visible surface on the phone. Black Titanium (grey) is one of Apple's finishes for this model, so it matches an original chassis.
