Do I need new adhesive every time?

Yes. The original is destroyed on removal, and a gap anywhere around the perimeter is an open path into the phone.

Does it restore water resistance?

It restores a proper physical seal, which is what you want. Treat any opened phone as no longer holding its factory rating whatever adhesive you use.
