Focus motor lens assemblies — the lens plus the voice-coil motor that moves it. When the motor fails you get a recognisable set of symptoms: focus that hunts and never settles, a rattle when the phone is shaken, or a buzz with a permanently blurred image.
The sensor behind the lens is usually undamaged, which is why replacing the lens and motor rather than the whole camera module makes sense. Note that some movement is normal on cameras with optical stabilisation — it is a loud rattle combined with a focus fault that indicates failure.
Likely, if focus also hunts or never settles. A small amount of movement is normal on cameras with optical image stabilisation, so it is the rattle plus the focus fault together that points at failure.
If the camera still captures an image but will not focus, the lens and motor assembly is the cheaper and usually sufficient fix. A dead or black camera needs the module.
